We can help if you are resident in Milton Keynes or are registered with a Milton Keynes GP.
This free service for all adults 18+ provides practical and emotional support for up to six weeks after leaving hospital, attending A&E or having treatment as a day patient.
If someone is unable to leave hospital because they have no family or other support locally, where it is appropriate, we can take them home and provide support to enable them to stay at home.
