Register for online services

Use this service to register to use GP online services.

You can use this service if you:

• are registered at the surgery

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• photographic identification and proof of address

Once you have completed this form and then visit the practice, bringing with you two forms of identification. One of these items should include your photograph.

We will then issue you a username and password.
